| Once
you have registered for a Reach Workcamp, there are a few forms that need
to be filled out to help us prepare for your arrival at camp. Below you
will find the necessary forms to be completed by your youth and adults as
well as some information concerning these forms. You should receive a copy
of each one of these forms by mail in the Leader Notebook after we have
received your registration, but if you need extras, just click on the form
you need and then print the form to mail back to us. Please be sure
to write your Church ID (ex. 09PRGCONNY2) on the bottom of each form before
you make copies so we can identify which church they are for.
